This survey consists of 35 open- and closed-ended questions and will take between five (5) to ten (10) minutes. You are being asked to participate because your opinion as a member of the Northeast Ohio theatre-going community is valuable to us at convergence-continuum. To participate in the survey simply click the “Take the Survey” link on the previous page; to decline, simply close this browser. Electronically submitting your completed survey connotes your consent to be a participant in our survey, and you will, at this time, be able to register to enter our drawing (see details).

Every effort has been taken to protect your identity as a participant. No survey respondent will be identified. All data collected via the survey will only be reviewed by members of convergence-continuum. BY NECESSITY, identifying personal information is collected for the contest. Please note, however, there is no way for convergence to connect your contest entry with your survey responses. Contest information will be stored securely on one machine and will not be made available to anyone outside of convergence-continuum.

There are no risks anticipated should you participate in this survey. This survey is designed to benefit convergence-continuum and, as an extension, the theatre-going community by helping us to gain new knowledge and to provide you with high quality theatre experiences. Your participation is important in helping us to reach the above-stated outcome.
